Ablation of the epicardial substrate in ventricular tachycardia associated with structural heart disease: outside in or inside out?

چکیده
Percutaneous epicardial access, introduced by Sosa et al. (1) in the mid-1990s, provided an important diagnostic and therapeutic approach to the recognition and management of scar-related epicardial ventricular tachycardia (VT). Since that time, the need for epicardial ablation has been reported in 10% to 15% of patients with ischemic cardiomyopathy (ICM) (2–4), 25% to 50% of patients with nonischemic cardiomyopathy (NICM) (4–8), and 30% to 40% of patients with arrhythmogenic right ventricular cardiomyopathy (ARVC) (4,9–12). Controversy persists with regard to the timing of epicardial intervention (initial procedure or only after unsuccessful endocardial approaches) and the extent of epicardial ablation required (focused regional ablation targeting specific VT morphologies or elimination of all epicardial late or abnormal potentials [13,14]). [Percutaneous epicardial radiofrequency ablation of idiopathic ventricular tachycardia].
We describe 3 patients with ventricular tachycardia in whom epicardial ablation was done after prior attempts of endocardial ablation had failed. Clinical ventricular tachycardia originated at the right ventricular outflow tract in one patient and near the mitral annulus in another patient. In these two cases ventricular tachycardia was mapped and successfully ablated with a percutaneous subxip...

BACKGROUND The epicardial location of an arrhythmia could be responsible for unsuccessful endocardial catheter ablation. METHODS AND RESULTS In 48 patients referred after prior unsuccessful endocardial ablation, we considered percutaneous, subxiphoid instrumentation of the pericardial space for mapping and ablation. Thirty patients had ventricular tachycardia (VT), 6 patients had a right- and...

BACKGROUND The complexity of reentrant circuits related to ventricular tachycardias decreases the success rate of radiofrequency ablation procedures. OBJECTIVE To evaluate whether the epicardial mapping with multiple electrodes carried out simultaneously with the endocardial mapping helps in ablation procedures of sustained ventricular tachycardia (VT) in patients with nonischemic heart disea...
متن کاملVentricular tachycardia in ischemic cardiomyopathy; a combined endo-epicardial ablation as the first procedure versus a stepwise approach (EPILOGUE) – study protocol for a randomized controlled trial
BACKGROUND The role of epicardial substrate ablation of ventricular tachycardia (VT) as a first-line approach in patients with ischemic heart disease is not clearly defined. Epicardial ablation as a first-line option is standard for patients with nonischemic dilated cardiomyopathy and arrhythmogenic right ventricular cardiomyopathy. Several nonrandomized studies, including studies on patients w...
متن کاملEpicardial substrate and outcome with epicardial ablation of ventricular tachycardia in arrhythmogenic right ventricular cardiomyopathy/dysplasia.
BACKGROUND Efficacy of endocardial ventricular tachycardia (VT) ablation in arrhythmogenic right ventricular cardiomyopathy/dysplasia may be limited by epicardial VT, right ventricular thickening, or both. We sought to characterize the endocardial versus epicardial substrate, measure right ventricular free wall thickness, and determine epicardial ablation efficacy in patients with right ventric...
